We’re excited to share that our enhanced patient portal experience, My Portal by Dignity Health, is now available, offering a more convenient way to access and manage your health needs between appointments more efficiently  – all from one central place.

[H2] My Portal by Dignity Health features

My Portal by Dignity Health was created with you in mind and designed to make navigation more intuitive. All features are easily accessible for a more intuitive experience , reducing clicks and saving you time.
With My Portal by Dignity Health, you can easily:
Search for current providers. To view all your current providers, click on “Appointments” and, using the “filter” feature, narrow down your search by provider name or appointment type.
Book appointments with ease. In most cases, you can schedule and reschedule appointments within My Portal by Dignity Health by looking up “past” or “canceled” appointments.
Find and choose a new doctor. If you need to find a new doctor and schedule an appointment, you don’t have to leave the portal. Click “Find a Doctor and Schedule” to find a doctor near you and schedule an appointment – all within My Portal by Dignity Health.
“Click to get directions” and “click to call.” You can call your doctor’s office with a single click and quickly navigate to your provider’s location using the maps app of your choice.
Add appointments to your calendar. Keeping track of your appointments is easier than ever. You can add upcoming appointments to your preferred calendar app with the click of a button.
Search for “past” or “canceled” appointments. Unsure if you’re due for a checkup or a screening? Viewing “past” and “canceled” visits is easier than ever. Just click on “Appointments” and select “past” or “canceled” from the drop-down menu to view precisely what you need in seconds.
View alerts and notifications. Notifications are displayed on the homepage of My Portal by Dignity Health. You’ll get alerts for new lab results and pending patient portal invitation(s) for your dependents (if you have any), with the added benefit of resending them to yourself instantly.

[H2] Getting started is as easy as 1-2-3!

Step 1: Set up your account (for new users only).
(Former My Care and existing My Portal users can skip this step).
Look for a patient portal invitation from Dignity Health ([email protected]) sent to the email you provided during your visit or after scheduling your appointment.
Follow the steps in the email to create your account.
Step 2: Download the My Portal by Dignity Health app.
(Existing My Portal users can skip this step).
After setting up your account, you can either download My Portal by Dignity Health app from the Apple App Store or Google Play, or visit dignityhealth.org and click on “My Portal login.”
Step 3: Log into My Portal by Dignity Health
Log into your patient portal using your existing credentials (former My Care and existing My Portal users only) or the email address and password you chose during your account set-up process (new users).
Log in through the mobile app or visit dignityhealth.org and click on “My Portal login” in the upper-right corner to log  in using your new or existing credentials.

[H2] My Portal by Dignity Health is replacing My Care

In simple terms, My Portal by Dignity Health is replacing My Care to serve you better. The enhanced My Portal by Dignity Health is part of our commitment to helping you better manage your health between visits, track appointments, and stay on top of preventive and screening services.

My Portal by Dignity Health replaced My Care, effective February 14, 2024, and became your go-to health management tool. The My Care mobile app will be officially retired as of April 16, 2024. After this date, My Care users won’t be able to get into the My Care mobile app.

My Care mobile app users:
Download the My Portal by Dignity Health app starting February 28, 2024, from the Apple Store or Google Play and log in using your existing username and password. After April 16, 2024, My Care users trying to log into the mobile app will see an alert on the app’s home screen informing them that the My Care app is no longer supported and all features are disabled. Users will then be directed to download the My Portal by Dignity Health app from the Apple Store or Google Play to continue to access their health information. My Care mobile app users will be unable to dismiss the alert message to enter the app.
My Care web-based users:
No action is required. The login process and your credentials will remain the same.

There will be no changes to your account access or data. Your My Care data will be migrated to My Portal by Dignity Health.

You will no longer use your My Care account to access your portal. If you are searching for information about historical visits or the care you received on or before May 31, 2021, please access your health records here. This link will be valid through December 31, 2024.

[H2] Getting started with My Portal by Dignity Health

You do not need to create a new account if you’re a My Care or My Portal user.

(Only new users need to enroll for portal access)
Setting up your account is the first step. To start the set-up process, look for a patient portal invitation sent to the email you provided to the hospital or clinic during your visit or after scheduling your appointment and follow the steps to set up your account. Talk to your doctor’s office front desk staff if you haven’t received a patient portal email invitation.

(For existing MyCare or My Portal users)
Simply log into the My Portal by Dignity Health mobile app or visit dignityhealth.org and click on “My Portal login” in the upper-right corner to log in using your existing credentials.
After logging into dignityhealth.org:
Click “Continue to My Portal” for instant access to your My Portal by Dignity Health dashboard.


OR
If you encounter the 'Welcome to your dashboard' page, select your care region, and click 'View' to access your records. Please follow the on-screen prompts if the 'View' option is not available.

Our My Portal by Dignity Health platform is designed to maintain data integrity and a high level of confidentiality regarding patients' data.

The My Portal by Dignity Health web-based application is available on all current browser and OS versions and standard devices, such as desktops, tablets, and mobile phones. Beginning February 14, 2024, you will be able to download the My Portal by Dignity Health mobile application.

[H2] Troubleshooting

If you’re getting a “404 Bad Request” error, you may have selected the wrong geographic region (care site). Try selecting the geographic region (care site) based on where you received care. If the problem persists, call (844) 274-8497.

We constantly assess the usage and impact of our services to ensure we allocate resources effectively and prioritize functionalities that benefit most users. In light of low usage, we have removed the Care Team feature to focus on delivering the best experience. You can still view and search for current providers within My Portal by Dignity Health. Simply click on Appointments and, using the new “filter” option, narrow down your search by provider name or appointment type.

If you need to reset your password, you can conveniently do it within My Portal by Dignity Health. Simply click on the Forgot Password button and follow the instructions sent to your email or phone number to regain access to your account.
If your account is not active or you need help adding an authorized representative, resending a pending or expired patient portal email invitation, or have other login problems, My Portal by Dignity Health offers technical support 24 hours a day, seven days a week by telephone, toll-free at (844) 274-8497.
