[H2] Request For Transcript/Records Form
All inquiries including graduation verification, transcript, shot records,IEP, ETR, and cumulative folder must be requested online using our new online student record request system. Fax, phone calls, and mail are not accepted.
Students who have graduated from Dayton Public Schools may request a duplicate diploma and/or diploma cover. Transcript processing time may vary from 10 - 30 business days.

NON-GRADUATE RECORDSAll schools maintain records for their current students. A student's record follows them to each Dayton Public School building they attend.
Open schools also maintain records for students who are not actively attending Dayton Public Schools. Each building only keeps 5 years of records in their building. Requests for those records should be directed to the respective school.
GRADUATE RECORDSHigh schools that are open maintain records for their graduates. Each building only keeps 5 years of records in their building. Requests for those records should be directed to the respective school.
CLOSED DPS AND CHARTER SCHOOLSRecords for schools must be requested online. In the case of charter schools that have closed, the district can only supply records that were provided to the district upon the school's closure.Where are records located?
Records for closed schools are the responsibility of Central Records.
Records for schools that are still open are at the schools. Schools only maintain 5 years of records.
Some inactive records for students born 1/1/70 or later are managed by the Student Enrollment Center.
How can I get my records?Click here to Request a Record or Duplicate Diploma OnlineCan I get a copy of my husband's (child's/parent's) record?No. The 1974 Privacy Act prohibits the release of a record without the authorization of the individual. The individual may designate someone else to pick up the transcript by providing a written note signed and dated by the individual.Will you accept a faxed release?No. Requests must be submitted online.Can you tell me over the phone if someone graduated from a specific high school?No. Requests must be submitted online.Can I request a copy of my shot record?Yes. Please make this request online. If there are no shot records on file, you will receive a refund. Health/medical records, visual/hearing screenings and immunization records are not required by law to be a part of the student's permanent record.

Welcome To Dayton Public SchoolsAttention: Parents of Incoming Kindergarteners for the 2026–2027 School Year — This Message Is for You!
Important Update
Beginning in March 2026, Ohio law (Ohio Revised Code 3321.01) will implement a single, statewide kindergarten age requirement. To enroll in kindergarten, a child must be five (5) years old on or before the first day of instruction.
For DPS, this means children who turn five on or before August 19, 2026, are eligible to enroll in Kindergarten for the 2026–2027 school year. If your child’s fifth birthday falls after August 19, 2026, they will need to wait and begin kindergarten the following school year. Please contact us if you have any questions regarding eligibility.Attention: 2026-2027 Enrollment
K-12 Enrollment opened on March 23, 2026, for the 2026-2027 school year. Appointments are required for Kindergarten students.
PreK Enrollment opened on April 06, 2026, for the 2026-2027 school year. Appointments are required for PreK students.

[H3] Gather The Required Enrollment Documents
Gather the required documents in order to enroll your students at Dayton Public Schools. The list is as follows –
Parent/Guardian Identification (valid driver's license, state ID, passport, I-94 card, green card, or community ID). A legal guardian must provide legal documentation of guardianship (birth certificate or custody documents)
Child's birth certificate (or child's passport if the parent(s)/guardian(s) name is included)
Child's shot records (if available but not required for enrollment) > VIEW IMMUNIZATION SUMMARY
Proof of Address
View Acceptable Proof OptionsProof of Address must include the custodial parent/legal guardian’s first and last name and complete address. Documents must be dated within 60 days of the appointment.

[H3] Schedule An Appointment
There are three ways to finish enrolling your child in Dayton Public Schools:
Traditional - In-person appointment
Hybrid - Download forms from above or complete the online form and schedule an in-person appointment
Virtual - Complete the online form, upload documents, and schedule a virtual appointment
If you choose to begin the online enrollment process above, all required enrollment documents must be legible and uploaded as directed prior to your appointment. To complete the online enrollment process, you must schedule and attend a virtual or in-person appointment.
If you are unable to upload any of the required documents or provide information for any of the mandatory fields, please schedule an in-person appointment.
Please note: Enrollment appointments are set for 20 minutes each. If you are enrolling more than one student, you will need to schedule an additional 20-minute appointment for each student.

[H5] Transfers
Transfers between DPS schools are ONLY accepted once a school year. Please visit the Student Enrollment Center from February 1st – March 31st. Walk-ins are welcome. All transfer requests submitted will be processed for the upcoming school year. You will receive a confirmation letter if your transfer was approved or denied.

How do I resolve a complaint or concern?DPS employees strive to provide excellent customer service and are happy to help resolve any issues you may be experiencing.
We ask that you first contact the principal or department head to work to resolve the issue. (Contact information for principals and department heads can be found on school pages or the Staff Directory.)
If you cannot resolve it at that level, please contact our Parent Ombudsman, Sharon Goins, at sgoins@daytonpublic.com, or call 937-542-3524.

How do I enroll my student at Dayton Public Schools?Enrolling in our district is as easy as ONE, TWO, THREE! There are three ways to enroll your child in Dayton Public Schools:
Traditional (in-person appointment)
Hybrid (Download forms or complete the online form and schedule an in-person appointment)
Virtual (Complete the online form, upload documents, and schedule a virtual appointment)
To view the required enrollment forms and to schedule your appointment, head to the Enrollment page on our website!How do I transfer my student to a different Dayton Public School?Transfers between DPS schools are ONLY accepted once a school year. Please visit the Student Enrollment Center from February 1st – March 31st. Walk-ins are welcome. All transfer requests submitted will be processed for the upcoming school year. You will receive a confirmation letter if your transfer was approved or denied.I live outside the District but want to enroll my student in a Dayton Public School. How do I do this?Open enrollment is held annually each spring, typically in March. Please make an appointment to enroll your student at that time. For questions, please call the Student Enrollment Center at 937-542-5555.I’m interested in enrolling my student and would like to tour a Dayton Public School. How can I set this up?We would love to welcome you and your family to DPS! Please contact the school you are interested in directly to schedule your tour.I want to enroll my student in a district-wide specialty school. How do I do this?Specialty schools include: David H. Ponitz CTC, Meadowdale CTC, Stivers School for the Arts, River’s Edge Montessori, and Charity Adams Earley Girls Academy.
Certain specialty schools, such as Ponitz, Meadowdale, or Stivers require an application (CTCs) or audition (Stivers). Other specialty schools may have a waiting list. Please contact Student Enrollment for more information at 937-542-5555.
Please note that CTC applications are available each fall for the following school year. If there are more applicants than seats available for any CTE program, students will be selected through a random lottery system.

Click here to visit the Stivers webpage.How do I withdraw my student?Those currently attending a DPS school must go to that building to officially withdraw a student.How do I homeschool my student?In Ohio, parents interested in educating their children at home may do so by notifying their local school district superintendents and meeting the requirements of Ohio law to home-school.
Parents who live within the Dayton Public School District and are interested in homeschooling their children should complete the application below and review the Frequently Asked Questions available on the Ohio Department of Education’s website.
To homeschool your student, you will need to fill out the Homeschool Intent Form found here. Any additional homeschool questions can be directed to studentenrollmentcenter@daytonpublic.com.

My student needs transportation to school. How do I get a bus?You will need to fill out the Bus Rider Registration form. The bus rider registration form can be found online here. If your student is on an IEP, please reach out to the Office for Exceptional Children (Special Education) for assistance. Any additional questions about transportation can be directed to their office at 937-542-4010.Is my student eligible for yellow bus transportation?Yellow bus transportation is provided to all K-8 students who meet the following eligibility requirements:
Live more than 1.5 miles from his/her school, OR
Has a route to school affected by one of four hazards (river, active railroad tracks, bridge with no sidewalks, and major highways), OR
Has transportation specified in his/her IEP.
A transportation request for a non-DPS student may be made through the school he/she attends.
Live in the same transportation zone as the school he/she is attending.

I need to request my transcript, diploma, or other student records. How do I do this?Dayton Public Schools uses a safe and secure platform called ScribOrder to complete all student records requests. Please submit your request for transcripts here.I am a representative from a K-12 school district and need to transfer a student’s records. How do I do this?Dayton Public Schools uses a safe and secure platform called ScribOrder to complete all student records requests. When submitting your request, there is a tab along the side for K-12 School Districts. Please select that section and submit your request here.How do I submit a public records request?Requests must be made in writing and should state that the information is being sought under the Ohio Public Records Act. Please provide a brief description of the public records being requested and be as specific as possible.

How do I update my address with Dayton Public Schools?Please visit your school or the Student Enrollment Center and bring a new proof of address document. The Student Enrollment Center is located at 136 S. Ludlow St. and can be reached at 937-542-5555.
Proof of Address must include the custodial parent/legal guardian’s first and last name and complete address. Documents must be dated within 60 days of the appointment.

My student is going to be absent from school for a doctor’s appointment. Who do I contact?Please call your school to report in advance that your student will be absent. Please provide a doctor’s note to excuse the absence as soon as possible.I received a notice that my student was absent, but they were in school. Who do I contact?Please contact your child's school and request to speak with the Attendance Clerk who can verify with your child's teacher if they are or were in class today.Why did my student receive an excessive absence notice when they have medical and excused absences on file?Excessive absences include ALL absences, even those that are excused. By law, the District is required to send the notice in accordance to House Bill 410.I received notice that my student has had excessive absences or is considered truant. Who do I contact?Please contact your child's school and request to speak with the Attendance Clerk or Attendance Coordinator/Student Success Coach assigned to your child's school. You can also request a meeting with your child's school Attendance Clerk or Attendance Coordinator/Student Success Coach.What is the difference between chronic absenteeism, excessive absences, and habitual truancy?These three terms all deal with student absences, but differ slightly based on the number of hours a student has been absent and whether it was an excused absence. How can my student get a work permit?Children under the age of 18 can obtain a work permit in the state of Ohio. There are two steps to completing a work permit: the pre-application and the physician’s certificate.
Students must first complete the pre-application. There are portions for the student, parent/guardian, school, and employer to complete.
The employer must provide a federal ID number and the number of hours to be worked in order for the application to be processed.
Students must then have a physician complete the physician’s form certifying that the student is physically able to perform the work.
If a student is an athlete and has a current physical on file with the district, this can be used in lieu of having a physician complete a form.
The student must then take both completed forms to the school. The school will then provide an Age and Schooling Certificate.
It will typically take 24 hours for the school to generate this certificate.
The student must then bring the Age and Schooling Certificate to their employer and are now eligible to begin working.
Please note that work permits are non-transferable. If a student changes jobs, a new application must be completed.
